How To Verify Your Relationship With DES
The relationship of the specified relative to each child for whom they are applying to receive TANF Cash Benefits or Medical Assistance, must be verified. Use of more than one document to verify relationship may be needed.
The following records may be used:
- Adoption records
- Baptismal records (when issued prior to age 5)
- Bio Data Sheet provided by Refugees
- Bureau of Vital Statistics
- Census records
- Certificate of Indian Blood (CIB)
- Certified birth certificate
- Court records
- Church records including statement from priest, etc.
- Arizona Health Care Cost Containment System (AHCCCS) Deemed Newborn Report (CR600)
- Acknowledgement of Paternity form (CS-127)
- Genetic testing results
- Hospital or public health records of birth
- Indian census records
- Insurance policy
- Juvenile court records
- Martial Status and Family Profile (Issued by the Navajo Nation)
- Military records
- Minute entries for Kinship Foster Care Cash Assistance applicants
- Outpatient care records maintained by a hospital, clinic, or doctor
- Paternity records
- Records maintained by IV-D agency (Dept. of Child Support Enforcement)
- School records
- School attendance verification
In addition to the documents previously listed, the following are acceptable verification of relationship for MA purposes:
- Father admits, in writing, to paternity of the child to an employee of DES\FAA
- Correspondence from an individual to the child, or to others about the child, referring to the child as his own.
