If Dr. Cotty has requested samples from you, his lab will send a package containing Red and White labels that say “DELIVER TO U.S. DEPARTMENT OF AGRICULTURE.” These are needed to ship the samples and/or cultures to our facility in Tucson Arizona.
Please retain a portion of each sample or a transfer of each culture in the event the packages are lost.
Samples must be double wrapped (i.e. in a plastic bag within a plastic bag) and shipped within nested boxes (a smaller box within a larger box – see diagram) or nested containers or nested pouches.
Inside box should contain:
- The double-wrapped samples and/or cultures.
- A copy of the 5 page paper permit in a pouch (permit and pouch provided by Cotty’s Lab).
- A Fedex Shipping Label addressed to Dr. Cotty (provided by Cotty's Lab).

Outside box should contain:
- The inside box.
- DHLInternational Waybill addressed to the U.S.D.A APHIS (Waybill and pouch provided by Cotty’s Lab).
- In a separate pouch, attached to the outside of the outside box, a copy of the permit (yes, the permit should be attached to the outside box and in the inside box). Depending on size of box or envelop, you can attach it in a manner that seems appropriate (permit and pouch provided by Cotty’s Lab).
- The Red and White label provided by Cotty’s lab must be placed on top of the outside package. This label must not be obscured by the DHL Waybill.
